- For each show you are ordering above:
Add up all amounts in the Total Deposits column and enter that amount in the box next to "Total Amount".
Fill in the rest of your information, especially your email address if you have one.
Indicate your acceptance of our Terms of Service (below) by signing your name in the indicated place.
If paying by credit card be sure to enter the the 3 digit code found on the back of your card
- Fill in the number of spaces you want.
- Multiply the number of spaces times the amount listed for show fee or deposit.
- Multiply the number of spaces times the amount for electricity (if needed).
- Add the previous two calculations together and enter the result in the "Total Deposit" column.

Important Note Electricity usage will be checked daily at the show. If you did not pay for it when you registered you will be charged for the electric plus a $5.00 late fee.

TERMS OF SERVICE
Carousel Craft Shows is not responsible for merchandise belonging to vendors and is exempt from any liabilities as a result of participation in a Carousel Craft Shows event. Leaving a show early will result in disbarment from future shows and forfeiture of any deposits on future shows. If you require confirmation please call 651-249-9772 five days after mailing in this form. To cancel a show, notify us within 30 days of the show. Unused deposits will be applied to future shows, if approved. No refunds on deposits. Not showing up for a show once applied for will result in forfeiture of deposits and the balances are still due and must be paid. Your space(s) ordered by deposit will only be held if the balance of the show is paid for within 10 days of the show. NO EXCEPTIONS. Set up the day before is recommended. Set up times are listed with each show. No moving in at all after the show starts.
